Vehicle Investigation

The vehicle investigation section allows a user to input the results of vehicle investigations performed for a specific vehicles after incidents. Vehicle Investigations will need to occur when a company employee is involved in a vehicle incident.

  1. Vehicle Accident is when vehicle is damage or someone is injured.
  2. Vehicle Complaint (Near Miss) is when no employees are injured or vehicle is damaged.

These vehicle investigation tasks is set up in five sections

  • Unsafe Acts
  • Unsafe Conditions
  • Personal Factors
  • Root Cause
  • Prevention

Once a vehicle investigation is submitted:

  • An email can be sent to the person in charge of the investigation and vehicles. (For more information about email alerts check the email alerts section of the manual)
  • Vehicle Net Statistics are updated for number of vehicle investigations completed for companies, employees, and vehicles.

Configuring Vehicle Net Vehicle Investigation Task Lists

These vehicle investigation tasks is set up in five sections; Unsafe Acts check list, Unsafe Conditions check list, Root Cause check list, Personal Factor check list, and Prevention check list. Companies can build their vehicle investigation task lists from default tasks SafetyNet provides. Companies may need to alter their vehicle investigation task lists as things change over time, as vehicle regulations or vehicles being used by companies have change

The vehicle investigation section is only available to users that have the vehicle investigation section turned on for their users group. To change users access to vehicle investigations, management and office staff can change access in Application Section to view the information about the Application Section Click Here

1.Add Vehicle Investigation

Steps
  1. From the Main Menu screen, tap the After Incident button.
  2. Then tap on the Vehicle Investigation button.
  3. This opens the Add Vehicle Investigation entry screen.
  4. Date: The date field allow field staff to choose the date the investigation occurs.
    1. Tap the Date button to open the Choose Date screen.
    2. Scroll the calender screen and select the date from the calender.
  5. Time: The time field allow field staff to choose the time the investigation occurs.
    1. Tap the Time button to open the Choose Time screen.
    2. Scroll the clock screen and select the time from the clock.
  6. Investigation Type select box.
  7. Investigation Types are listed in descending order with the investigation type name.

    1. Scroll the list of investigation types until the desired investigation type is displayed on the screen.
    2. Tap the desired meeting type to select it.
  8. Work Order select box.
  9. Work orders are listed in descending order with the work order number in black text, and the scope of work in black text.

    Note: New work orders are not automatically added to the list when they are created, so it may be necessary to update the list click here to view information about updating for the desired work order to be included in the list.

    1. Scroll the list of work orders until the desired work order is displayed on the screen.
    2. Tap the desired work order to select it.
  10. Tap the Save Vehicle Investigation button to add the Vehicle Investigation.
    • A pop-up message that reads Vehicle Investigation Added indicates that the Vehicle Investigation is ready to have details to be entered and checklists of investigation tasks checked off.

2.Look Up Open Vehicle Investigation

Once in the Vehicle Investigation section

Steps
  1. Tap the second tab on the menu, the Vehicle Investigation view.
  2. Vehicle Investigations are divided into three categories
    1. Medical Incident
    2. Near Miss Incident
    3. Property Damage / Spill Release
  3. Tap the category your investigation is listed under.
  4. Scroll the list of Vehicle Investigations until the desired Vehicle Investigation is displayed on the screen. Listing the Vehicle investigation date in red text, the work order number in black text.
  5. Tap on the Vehicle Investigation you would like to view. This loads the Vehicle investigation edit screen.

3.Edit Vehicle Investigation

The first step in completing a Vehicle Investigation is completing the details of the investigation; including date, location of the incident, road conditions, and weather conditions.

Once on the Vehicle Investigation you would like to Edit, on the details tab.

  1. Incident Date: The date field allow field staff to choose the incident date the safety investigation occurs.
    1. Tap the Date button to open the Choose Date screen.
    2. Scroll the calender screen and select the date from the calender for the incident date.
  2. Incident Time: The time field allow field staff to choose the incident time the safety investigation occurs.
    1. Tap the Time button to open the Choose Time screen.
    2. Scroll the clock screen and select the time from the clock for the incident time.
  3. Report Date: The date field allow field staff to choose the report date the safety investigation occurs.
    1. Tap the Date button to open the Choose Date screen.
    2. Scroll the calender screen and select the date from the calender for the report date.
  4. Report Time: The time field allow field staff to choose the report time the safety investigation occurs.
    1. Tap the Time button to open the Choose Time screen.
    2. Scroll the clock screen and select the time from the clock for the report time.
  5. Investigation Date: The date field allow field staff to choose the investigation date the safety investigation occurs.
    1. Tap the Date button to open the Choose Date screen.
    2. Scroll the calender screen and select the date from the calender for the investigation date.
  6. Investigation Time: The time field allow field staff to choose the investigation time the safety investigation occurs.
    1. Tap the Time button to open the Choose Time screen.
    2. Scroll the clock screen and select the time from the clock for the investigation time.
  7. Location of Incident: The Location of incident field should describe the location of the incident. The character limit of this field is 250.
  8. Purpose of Trip: The Purpose of Trip field should describe the purpose of the trip when the incident occurred. The character limit of this field is 250.
  9. Weather Conditions: The Weather Conditions field should describe the weather conditions when the incident occurred. The character limit of this field is 250.
  10. Road Conditions: The Road Conditions field should describe the road conditions when the incident occurred. The character limit of this field is 250.
  11. Tap Save Vehicle Investigation button to save any changes to the Vehicle Investigation Details.
    • A pop-up message that reads Vehicle Investigation Details have been Saved indicates that the Vehicle investigation details have been saved.

4.Edit Vehicle Investigation Incident Details

The second step in completing a Vehicle Investigation is completing the details of the investigation; including information about the incident.

Once on the Vehicle Investigation you would like to Edit, on the incident info tab.

Steps
  1. Your Speed: The Your Speed field should describe the speed of the driver at the time of the incident. The character limit of this field is 50.
  2. Your Direction: The Your Direction field should describe the direction the driver was traveling at the time of the incident. The character limit of this field is 50.
  3. Other Speed: The Other Speed field should describe the speed of the other driver at the time of the incident. The character limit of this field is 50.
  4. Other Direction: The Other Direction field should describe the direction the other driver was traveling at the time of the incident. The character limit of this field is 50.
  5. Police Officer Name: The Police Officer Name field should record the name of the police officer involved in the incident. The character limit of this field is 50.
  6. Police Officer Phone Number: The Police Officer Phone Number field should record the phone number of the police officer involved in the incident. The character limit of this field is 50.
  7. Charges Laid: The Charges Laid field should describe the charges laid as a result of the incident. The character limit of this field is 250.
  8. Impaired: The Impaired field should be set to Yes if driver was impaired at the time of the incident.
    1. Scroll the list of Yes or No.
    2. Tap the desired selection.
  9. Impairment Description: The Impairment Description field should describe how the driver was impaired at the time of the incident. The character limit of this field is 250.
  10. Incident Responsibility: The Incident Responsibility field should describe who was responsible for the incident. The character limit of this field is 250.
  11. Seat Belt Worn: The Seat Belt Worn field should be set to Yes if the driver was wearing a seat belt at the time of the incident.
    1. Scroll the list of Yes or No.
    2. Tap the desired selection.
  12. Tap Save Vehicle Investigation button to save any changes to the Vehicle Investigation Details.
    • A pop-up message that reads Vehicle Investigation Details have been Saved indicates that the Vehicle investigation details have been saved.

5.Add Employee to Vehicle Investigation

Adding employees to the Vehicle Investigation is crucial to reporting accurately. Adding other field staff to the Vehicle investigation allows a group of workers to share and complete a Vehicle investigation as a group. Once a Vehicle investigation is submitted all employee's added to the Vehicle investigation will have access to the Vehicle investigation. Here are the steps on how to do it:

Once on the Vehicle Investigation you would like to Edit

Steps
  1. Tap the Employees icon at the top of the screen to open the Employees screen.
  2. Tap the Employees button to open the Employees screen.
  3. Tap the + icon in the top right corner to open the Employees list or tap Employees in the top left corner to return to the Employees screen.
  4. Employees are listed in descending order of last name.

  5. Scroll the list of employees until the desired employee is displayed on the screen.
    1. If the desired employee is in the list, tap the employee to add them to the Vehicle Investigation.
    2. A pop-up message that reads Employee Added indicates that the employee is now listed on the Vehicle Investigation Employees screen.

    3. If the desired employee is not in the list, tap + in the top right corner of the screen to open the New Employee screen.
      1. First Name: The First Name field should indicate the legal given name of the employee. The character limit of this field is 25.
      2. Last Name: The Last Name field should indicate the legal surname of the employee. The character limit of this field is 25.
      3. Email: The Email field should indicate the best (i.e., most often used for business purposes) e-mail address for the employee. The character limit of this field is 25.
      4. Phone Number: The Phone Number field should indicate the best (i.e., most often used for business purposes) telephone number (land line or cellular) for the employee. The character limit of this field is 13.
      5. Company: The Company field should indicate the name of the company for which the employee works. The character limit of this field is 75.
  6. Tap Save Task button to add the outside employee to the Vehicle Investigation.

6.Delete Employee from Vehicle Investigation

Once on the Vehicle Investigation you would like to Edit

Steps
  1. Tap the Employees icon at the top of the screen to open the Employees screen.
  2. Tap the Employees button to open the Employees screen.
  3. Scroll the list of employees until the desired employee is displayed on the screen.
  4. Long Tap the employee to bring up the delete screen for the employee.
  5. Tap the Delete button. The employee will be deleted.

7.Add Employee Signature

Each employee added to the Vehicle Investigation must provide an electronic signature to indicate that they taken part in the investigation.

Once on the Vehicle Investigation you would like to Edit

  1. Tap the Employees icon at the top of the screen to open the Employees screen.
  2. Tap the Employees button to open the Employees screen.
  3. Scroll the list of employees until the desired employee is displayed on the screen.
  4. Tap the desired employee to select them and open the Edit Employee screen
  5. Have the selected employee sign in the white space beneath the Please Sign Below label.
  6. Tap Save in the top right corner of the screen to update the Saved Signature field.
  7. The saved signature will appear below the signature entry screen.

18.Add Sequence of Events

Once on the Vehicle Investigation you would like to edit.

  1. Tap the Employee icon, the third tab. This opens the list of Employees, Sequence of Events, Witness Statement, Vehicle Involved, and Drawing.
  2. Tap the Sequence of Events button
  3. Tap the + icon in the top right corner to load the add sequence of events screen
  4. Fields
  5. Order of Events: The order of events field allows users to add sequence of events paragraphs in a specific order in any order they see fit.
  6. The Order of Events select box loads with plus one over the number of sequence of events paragraphs

    1. Scroll the list of order of events paragraph numbers until the desired order of events paragraph number is displayed in the select box.
    2. Tap the order of events paragraph number to select it
  7. Description of Events: The Description of Events field gives the user the ability to describe the sequence of events that has occurred. The user should enter a description of the injury. The character limit of this field is 500.
  8. Tap Save Sequence of Events button on the bottom of the screen to save the sequence of events.

19.Look Up Sequence of Events

Once on the Vehicle Investigation you would like to edit.

  1. Tap the Employee icon, the third tab. This opens the list of Employees, Sequence of Events, Witness Statement, Vehicle Involved, and Drawing.
  2. Tap the Sequence of Events button
  3. Scroll the list of Sequence of Events until the desired Sequence of Events is displayed on the screen. The order of events in blue text, with the description of events in black text.
  4. Tap on the Sequence of Events you would like to view. This loads the sequence of events edit screen.

20.Edit Sequence of Events

Once on the Sequence of Events edit screen

Fields
  1. Order of Events: The order of events field allows users to add sequence of events paragraphs in a specific order in any order they see fit.
  2. The Order of Events select box loads with plus one over the number of sequence of events paragraphs

    1. Scroll the list of order of events paragraph numbers until the desired order of events paragraph number is displayed in the select box.
    2. Tap the order of events paragraph number to select it
  3. Description of Events: The Description of Events field gives the user the ability to describe the sequence of events that has occurred. The user should enter a description of the injury. The character limit of this field is 500.
  4. Tap Save Sequence of Events button on the bottom of the screen to save the sequence of events.

21.Delete Sequence of Events

Once on the Vehicle Investigation you would like to edit.

  1. Tap the Employee icon, the third tab. This opens the list of Employees, Sequence of Events, Witness Statement, Vehicle Involved, and Drawing.
  2. Tap the Sequence of Events button
  3. Tap the Edit button in the top right corner to load the sequence of events paragraphs list screen
  4. Scroll the list of Sequence of Events until the desired Sequence of Events is displayed on the screen. The order of events in blue text, with the description of events in black text.
  5. Long Tap the sequence of event to bring up the delete screen for the sequence of events.
  6. Tap the Delete button. The sequence of events will be deleted from the vehicle investigation.

22.Add Witness Statement Employee

Once on the Vehicle Investigation you would like to edit.

Steps
  1. Tap the Employee icon, the third tab. This opens the list of Employees, Sequence of Events, Witness Statement, Vehicle Involved, and Drawing.
  2. Tap the Witness Statement button
  3. Scroll the list of Vehicle Investigation Employees until the desired employee is displayed on the screen.
  4. Tap the employees name to add a witness statement for selected employee.

23.Add Witness Statement

Once on the Vehicle Investigation you would like to edit.

  1. Tap the Employee icon, the third tab. This opens the list of Employees, Sequence of Events, Witness Statement, Vehicle Involved, and Drawing.
  2. Tap the Witness Statement button
  3. Scroll the list of witness statement for employees until the desired employee is displayed on the screen.
  4. Tap the Edit button in the top right corner to load the witness statement paragraphs list screen
  5. This loads the list witness statement paragraphs.
  6. Tap the + icon in the top right corner to load the add witness statement screen
  7. Fields

24.Look Up Witness Statement

Once on the Vehicle Investigation you would like to edit.

  1. Tap the Employee icon, the third tab. This opens the list of Employees, Sequence of Events, Witness Statement, Vehicle Involved, and Drawing.
  2. Tap the Witness Statement button
  3. Scroll the list of Witness Statement by employee until the desired Witness Statement is displayed on the screen. The employee name in black text.
  4. Tap on the Witness Statement, which will load the Edit Witness Statement screen.

26.Delete Witness Statement

Once on the Vehicle Investigation you would like to edit.

  1. Tap the Employee icon, the third tab. This opens the list of Employees, Sequence of Events, Witness Statement, Vehicle Involved, and Drawing.

  3. Scroll the list of Witness Statements until the desired Witness Statements is displayed on the screen. The witness statement in black text.
  4. Long Tap the witness statement to bring up the delete screen for the witness statement.
  5. Tap the Delete button. The witness statement will be deleted from the vehicle investigation.

27.Add Vehicle Involved

Once on the Vehicle Investigation you would like to edit.

  1. Tap the Employee icon, the second tab. This opens the list of Employees, Sequence of Events, and Witness Statement.
  2. Tap the Vehicle Involved button
  3. Tap the + icon in the top right corner to load the add vehicle involved screen
  4. Fields
  5. Vehicle: The vehicle field should be set vehicle involved in the incident. If a none company vehicle is involved, do not select a vehicle and fill in the rest of the fields.
    1. Scroll the list of vehicles until the desired vehicle is displayed in the select box.
    2. Tap the desired vehicle to select it

  9. Policy Number: The Policy Number field should be used to record the policy number of the insurance the vehicle involved is insured under. The character limit of this field is 500.
  10. Driver: The driver field should be set to the person driving the vehicle involved at the time of the incident.
    1. Scroll the list of driver until the desired driver is displayed in the select box.
    2. Tap the desired driver to select it
  11. Make of Vehicle: The Make of Vehicle field should be used to record the make of the vehicle involved. The character limit of this field is 500.
  12. Model of Vehicle: The Model of Vehicle field should be used to record the model of the vehicle involved. The character limit of this field is 500.
  13. Vehicle Year: The Vehicle Year field should be set to year of the vehicle involved.
  14. Users should enter a four digit number for the vehicle yea.
  15. Description of Damage: The Description of Damage field should be used to record any damage to the vehicle involved, which was caused be the accident. The character limit of this field is 500.
  16. Tap Save Vehicle Involved on the bottom of the screen to save the vehicle involved.

31.Save Vehicle Incident Drawing

Once on the Vehicle Investigation you would like to edit.

  1. Tap the Employee icon, the second tab. This opens the list of Employees, Sequence of Events, and Witness Statement.
  2. Tap the Drawing button, this loads the View Accident Sketch drawing.
  3. Tap the Edit button, in the top right corner. The edit accident sketch screen.
  4. Use your figures to sketch the incident on the
  5. Tap the Save button, in the top right corner. The edit accident sketch scree screen

21.Submit Vehicle Investigation

Once on the Vehicle Investigation Section.

  1. Tap the first tab on the menu, the Vehicle Investigation view.
  2. Tap the Submit Vehicle Investigation button.

22.Delete Vehicle Investigation

Once in the Vehicle Investigation section

  1. Tap the second tab on the menu, the Vehicle Investigation view.
  2. Vehicle Investigations are divided into three categories
    1. Medical Incident
    2. Near Miss Incident
    3. Property Damage / Spill Release
  3. Tap the category your investigation is listed under.
  4. Scroll the list of Vehicle Investigations until the desired Vehicle Investigation is displayed on the screen. Listing the Vehicle investigation date in red text, the work order number in black text.
  5. Long Tap the vehicle investigation to bring up the delete screen for the vehicle investigation.
  6. Tap the Delete button. The vehicle investigation will be deleted.

23.View Closed Vehicle Investigation

After Vehicle Investigations have been closed out, they will be available to look at through the History view on the iPhone Application.

Once on the Vehicle Investigation section

Steps
  1. Tap the third tab on the menu, the History tab of the main menu view. This loads with a list of the closed out Vehicle Investigations listing the the date, work order number, customer and job site name of the Vehicle Investigation in black.
  2. Scroll the list of Vehicle Investigations until the desired Vehicle Investigation is displayed on the screen.
  3. Tap on the Vehicle Investigation you would like to view. This loads the first tab, the Vehicle Investigation details tab of the History view. It loads the following details including dates, and information about conditions at the time of the incident.
  4. Tap the second tab, the vehicle incident information of the History view. This information includes information about what was happening at the time of the incident.
  5. Tap the third tab, the Employee's tab of the History view. This loads with the Sequence of Events, Witness Statements, Vehicles Involved, incident drawing , and employee's names and their signatures of the employees assigned to the Vehicle Investigation.
  6. Tap the fourth tab, the Investigation Categories tab of the History view.The screen loads with the list of the 5 checklist categories of the Vehicle investigation. Tap on each category to view the investigation tasks.

24.Update Vehicle Investigations

To ensure field staff have the most up to date copy of the investigation tasks a company is using make sure to update your phone on a regular basis. For more information about updating your phone.

  1. From the Menu screen, tap the Updates button.
  2. Scroll the list of sections until you find the Vehicle Investigation button.
  3. Tap on the Vehicle Investigation button.
