Questionnaires

The questionnaire section is set up to allow company staff to create a questionnaire for a staff member to complete.

Staff can choose from company questionnaire, SafetyNet allows companies to create company questionnaires from many default questionnaires.

Some Safety Programs require that staff complete reviews of safety manual documents on some form of annual basis, including these reviews in the course of meetings helps get this procedure completed as part of regular meetings. Another advantage of reviewing safety manual documents in meetings is younger field staff can learn from the older field staffs experience when documents are discussed in a open environment.

SafetyNets Smartphone application allows field staff to complete questionnaires in the field and submit them to the system. For information about completing a questionnaires through the Smartphone click here.

Once a Meeting is submitted:

  1. 1. An Email can be sent to the employee to review the questionnaire and to the employee whom prepared the questionnaire. (For more information about email alerts check the email alerts section of the manual).
  2. 2. Safety Net Statistics are updated for the number of questionnaires completed and passed for companies and employees.

Setting up how often questionnaires should occur

How often questionnaires must occur is a function of your companies policy regarding questionnaires. To configure Safety Net for how often questionnaires should occur can be configured in the company safety settings.

Management, Office Staff and Safety Officers can edit how often questionnaires should occur in the Company section when they edit the Company Safety settings.

The questionnaire section is only available to users that have the questionnaire section turned on for their users group. To change users access to meetings, management and office staff can change access in Application Section to view the information about the Application Section Click Here

Important Notice!!!!

SafetyNet is set up so that users can take questionnaires online and other users can mark a questionnaire to determine if users passed the questionnaire. So users that are taking the questionnaire, set up as the Employee on the Questionnaire can edit their answer. The users set up as Prepared By user can edit the answer and the marks received for each question. This Manual will talk about the page from the Prepared By user perspective.

1.Search Questionnaires

Questionnaires can be searched by a combination of the 'Employee','Questionnaire Name','Assessment Method','Questionnaire Type','Prepared By' and 'Submission Status'. Users can narrow down the search results by choosing any combination of the six search criteria.

All search result can be sorted by clicking on the results headings; Date, Employee, Questionnaire name, assessment method, questionnaire type, prepared by and Status. Clicking on the same heading can do switching the results order from ascending to descending.

2.Delete a Questionnaire

Delete a questionnaire may have to be done if the report is corrupted or if questionnaire was added to the system incorrectly. Only questionnaires that have not been reviewed by management can be deleted.

To delete a questionnaire, click the 'Delete' link of the questionnaire you want to delete.

3.Look up a Questionnaire

To get to the edit screen of a questionnaire, click on the “Date” link of the questionnaire that you would like to edit.

To view the report in html click on the 'Html view' link of the questionnaire you would like to view.

To view a printable document file (PDF) view of the questionnaire report, click on the 'PDF view' link of the appropriate questionnaire report.

4.Add Questionnaire

To add a questionnaire, click on the 'Add Questionnaire' link.

On the next screen, enter information in the following fields:

Fields

Date: The date field defaults to today$s date. The date field is formatted in YYYY-MM-DD format and will not accept any other format.

Employee: By selecting a employee from the employee select box users can choose whom took the questionnaire. On the add questionnaire you can choose any active employee from the company.

Prepared By: By selecting a employee from the employee by select box users can choose whom is in charge of reviewing the questionnaire. On the add questionnaire you can choose any active employee from the company.

Click the 'Save Questionnaire' button to add the questionnaire.

5.Edit a Questionnaire

Once a user has clicked the 'Date' field, the Edit Questionnaire window opens

Fields

Employee: By selecting a employee from the employee by select box users can choose whom took the questionnaire. On the add questionnaire you can choose any active employee from the company.

Prepared By: By selecting a employee from the employee by select box users can choose whom is in charge of reviewing the questionnaire. On the add questionnaire you can choose any active employee from the company.

Once the information has been input,click the 'Save Questionnaire' button to ensure that any information changed is saved in the system.

6.Edit Long Answer Question

Once the edit questionnaire screen

Click the Edit link of a Question.

For the Employee taking the Questionnaire, the question, the long answer field will load. With any graphic which was added to the question.

For the Prepared By user of the Questionnaire, the question, the long answer field will load, and the marks given for the question. With any graphic which was added to the question.

Fields

Answer: Allows a user to input the answer to the question. The character limit of this field is 500.

Marks Given: Allows a user to input the number of marks given for the employees answer.

Once the long answer question information have been edited, click on the 'Save Long Question Answer' button.

7.Edit Multiple Answer Question

Once the edit questionnaire screen

Click the Edit link of a Question.

For the Employee taking the Questionnaire, the question, the multiple answers field will load. With any graphic which was added to the question.

For the Prepared By user of the Questionnaire, the question, the multiple answers field will load. With any graphic which was added to the question.

Fields

Multiple Answer: Allows a users to choose the answer from multiple answer select box.

Once the multiple answer question information have been edited, click on the 'Save Multiple Answer Question' button.

8.Submitting the Questionnaire

Once a questionnaire has been completed, it must be submitted for review by management or the employee whom prepared the questionnaire. To do this, click on the 'Submit Questionnaire' button. An email will be sent to the employee whom prepared the questionnaire.

9.Reviewing the Questionnaire

Once a questionnaire has been submitted management or a supervisor should review and sign off that questionnaire have been completed.

Once a questionnaire has been completed, it must be submitted for review by management. To do this, click on the 'Submit Questionnaire' button. An email will be sent to the employee whom prepared the questionnaire.
