The confined space entry report section allows users to create and record a confined space entry report. Confined space entry reports
need to occur when field staff work in a confined space.
Confined Space Entry Reports in R & T Electric Pro require
- Work Order: A work order is the identifying project that the field staff are working on. In R & T Electric Pro this work order is
only for work being done on a single job site. The work order will be identified by the scope of work.
- Field Staff: Field staff whom will be working on the job site need to be active in the R & T Electric Pro system. They will need to
- Enter time card entries for time spent working on work order for the day of the confined space entry report.
- Create hazard assessments and mark that the work involved includes work in a confined space.
- Each employee working needs to be added to the confined space entry report and sign off that they have reviewed the confined space entry report.*
*While it is possible to add a employee thru the Add Outside Employee feature on the confined space entry report, no safety statistics can be
created for the employee"s added this way.
Confined Space Entry Reports in R & T Electric Pro track
- Confined Space Details: Confined space details about where the work was done on site, what work was done and a description of the space being worked on are all recorded in confined space entry report.
- Employees: Which employees participated in confined space entry.
- Gas Monitoring Readings: Track which contaminants were test for and their corresponding readings.
- Entry/Exit Log: Entry and Exit logs for employees entering the confined space.
- Considerations: Considerations are the issues field staff consider while working in the confined space.
- PPE: The Personal Protective Equipment required in the confined space.
- Control Measures: The Control Measures that are put in place for field staffs safety while working in the confined space.
Safety Net Smartphone application allows field staff to complete confined space entry reports in the field and submit them to the system.

Once a hazard assessment is submitted:
- An email can be sent to the person in charge of the confined space entry reports. (For more information about email alerts check the email alerts section of the manual)
- Employee Timecard entries will be updated to show that a confined space entry report has been completed for the appropriate job site and day of work.
- Safety Net Statistics are updated for number of confined space entry reports completed for companies, customers, work orders and job sites.

Configuring Company Review Notes
Configuring a companies confined space review notes are the notes field staff should review as part of every confined space entry report, they are added to every confined space entry report in the SmartPhone application. For more

Creating Work Order
The Work Order section allows companies to create work orders to define and track where employee spend their time working and how many hazard assessments they complete. For more information about managing

1.Search Confined Space Entry Report |
|
Confined Space Entry Reports can be searched by a combination of customer, work orders, month and submission status. Users can narrow search results by choosing any combination of the four search criteria.
All search results can be sorted by clicking on the results headings Date, Work Order Number, Customer Name,Job Site Name,
and Status. Clicking on the same heading can do switching the results order from ascending to descending.

2.Delete Confined Space Entry Report |
|
Deleting confined space entry reports may have to be done if the confined space entry report was added to the system incorrectly. Only confined space entry reports that have not been reviewed by management can be deleted.
To delete a confined space entry reports click the 'Delete' link of the confined space entry report you want to delete.

3.Look Up Confined Space Entry Report |
|
To get to the edit screen of a confined space entry report, click on the date of the confined space entry report that you would like to edit.
To view the html version confined space entry report report in click on the 'Html view' link of the confined space entry report you would like to view.
From each html view you can also open the PDF version of the report by clicking on the PDF link on the top of the page.
To view a printable document file (PDF) view of the confined space entry report report, click on the 'PDF view' link of the appropriate confined space entry report.

4.Add Confined Space Entry Report |
|
To add a Confined Space Entry Report, click on the 'Add Confined Space Entry Report' link.
On the next screen, under Add Confined Space Entry Report heading, select:
Fields
Work Order: By selecting a work order from the select box, which loads with a work order number users can choose which work order they want to create a confined space entry report for.
Employee: By selecting a employee from the employee select box, which loads with the employees first name and last name users can choose whom conducted the confined space entry report.
On the add confined space entry report section you can choose any active employee from the company.

Then click the 'Add Confined Space Entry Report' button. The edit confined space entry report screen will load. From here, proceed to edit as per the instructions below.

5.Edit Confined Space Entry Report Details |
|
Once on the confined space entry report edit screen.
Fields
Work Order: By selecting a work order from the select box, which loads with a work order number users can edit which work order the confined space entry report is for.
Date of Entry: The date of entry field is the date the confined space entry work starts. The date field is formatted in YYYY-MM-DD format and will not accept any other format.
Time of Entry:The time of entry field is the time the confined space entry work starts. The hour select box functions in a 24 hour clock. The minutes select box is populated with 1 to 59.
Date of Exit: The date of exit field is the date the confined space entry work stops. The date field is formatted in YYYY-MM-DD format and will not accept any other format.
Time of Exit:The time of exit field is the time the confined space entry work stops. The hour select box functions in a 24 hour clock. The minutes select box is populated with 1 to 59.
Calibration Date: The calibration date field is the date the gas monitor was calibrated. The date field is formatted in YYYY-MM-DD format and will not accept any other format.
Scope of Work: The Work to Be Done field initially displays the scope of work from the work order and, therefore, may need to be refined to reflect the scope of work for the day/shift. The character limit of this field is 250.
Location of Space: The location of confined space field should identify the specific location of the confined space. The character limit of this field is 250.
Description of Space: The Description of Space field should describe the type of confined space being worked in. The character limit of this field is 250.
Atmosphere: By selecting a atmosphere from the atmosphere select box users can choose which atmosphere the best describes the conditions of the confined space.
Ventilation: By selecting a ventilation from the ventilation select box users can choose a type of ventilation to be used to service the confined space.
Emergency Number: The Emergency number field allows field staff to enter a emergency number to be called in case of a emergency. The character limit of this field is 20.
Radio Use: The Radio Use field can be set to Yes if radios are allowed to be used in the confined space for communication purposes.
Review Notes: The Review Notes field can be set to Yes if the the confined space review notes have been reviewed.
Click the 'Save Confined Space Details' button to ensure that any information changed is saved in the system.

6.Remove Confined Space Employee |
|
Removing a employee may be required if the wrong employee was added to a confined space entry report. If the employee did not participate in the confined space
entry report or did not work on the job site on the given day they should not be on the confined space entry report. Once a confined space entry report has been
submitted employees can t be removed from the confined space entry report.

Confined Space Employee: Select from the select box the employee to be removed.
click the 'Remove Employee' button.

7.Add Confined Space Employee |
|
Adding employees to the Confined Space Entry Report is crucial to reporting accurately. In order to have an accurate picture of the job site, adding employees to
the Confined Space Entry Report needed. While the confined space entry report was created, the first employee was added to confined space entry report. Adding other field staff to the
confined space entry report allows a group of workers to share the same confined space entry report. Once a confined space entry report is submitted all employee's added
to the confined space entry report will have access to the confined space entry report.

Confined Space Employee: Select from the select box the employee to be removed.

8.Add Temporary Employee to Confined Space Entry Report |
|
Once and a while a person may need to added to a hazard assessment, who is not a employee of your company. A subcontractor, or a customer employee are
some examples. Its still important that they review the hazard assessment. R & T Electric Pro allows field staff to add outside employees to a hazard assessment.
IMPORTANT NOTE Its very important that the add temporary employee function is not used to add a new employee to the company to a hazard assessment.
Temporary employee statistics are not tracked. So its very important to add a new employee to your company the proper way to R & T Electric Pro. For more information

Fields
First Name: The First Name field should indicate the legal given name of the employee. The character limit of this field is 25.
Last Name: The Last Name field should indicate the legal surname of the employee. The character limit of this field is 25.
Email: The Email field should indicate the best (i.e., most often used for business purposes) e-mail address for the employee. The character limit of this field is 25.
Phone Number: The Phone Number field should indicate the best (i.e., most often used for business purposes) telephone number (land line or cellular) for the employee. The character limit of this field is 13.
Company: The Company field should indicate the name of the company for which the employee works. The character limit of this field is 75.

9.Edit Confined Space Employee |

Once on the edit confined space entry report screen
Below the Confined Space Employees heading
Click the Employee name link of the employee you would like to edit.
Fields
Confined Space Role: By selecting a confined space role from the confined space role select box users can choose which confined space role for the employee that best suites their role in the confined space work.
Start Date: The start date field is the date the confined space entry work starts. The date field is formatted in YYYY-MM-DD format and will not accept any other format.
Start Time:The start time field is the time the confined space entry work starts. The hour select box functions in a 24 hour clock. The minutes select box is populated with 1 to 59.
Stop Date: The stop date field is the date the confined space entry work stops. The date field is formatted in YYYY-MM-DD format and will not accept any other format.
Stop Time:The stop time field is the time the confined space entry work stops. The hour select box functions in a 24 hour clock. The minutes select box is populated with 1 to 59.
Click the 'Edit Confined Space Employee' button.

10.Add Contaminant |

Once on the edit confined space entry report screen
Open the Gas Monitoring tab.
Below the Add Contaminant heading
Fields
Contaminant Name: The name of the contaminant. The character limit of this field is 150 characters.
Designation: The designation of the contaminant. The contaminant elemental designation. The character limit of this field is 25 characters.
Click the 'Add Contaminant' button.

To get to the edit screen of a Contaminant, click on the contaminant name of the contaminant that you would like to edit.

The contaminant and all related gas monitoring readings will be deleted from the confined space entry report.

14.Add Gas Monitoring Reading |
|
Once on the edit contaminant screen

Pre Purge/Post Purge: By selecting a pre purge or post purge from the purge select box users can choose if the reading is taken pre purge or post purge.
Reading:The reading field is the gas monitoring reading taken for the specific contaminant. This is a numeric value.
Reading Date: The reading date field is the date the gas monitoring reading is taken. The date field is formatted in YYYY-MM-DD format and will not accept any other format.
Reading Time:The reading time field is the time the gas monitoring reading is taken. The hour select box functions in a 24 hour clock. The minutes select box is populated with 1 to 59.
Reading By: By selecting employee from the reading by select box users can choose the employee whom takes the gas monitoring reading.
Click the 'Add Reading' button.

To get to the edit screen of a Gas Monitoring Reading, click on the reading date of the gas monitoring reading that you would like to edit.

17.Delete Gas Monitoring Reading |
|
To delete a Gas Monitoring Reading, click on the delete link of the gas monitoring reading that you would like to delete.
The reading will be deleted from the confined space entry report.

18.Add Calibration Report |
|
Once on the edit confined space entry report screen
Open the Gas Monitoring tab.
Below the Upload Calibration Report heading
Choose a File: to Upload.
Click the 'Upload Calibration Report' button.

19.Delete Calibration Report |
|
Clicking the delete link of a calibration record. Will delete the calibration record in the system.

20.Add Employee To Entry Log |
|
Once on the edit confined space entry report screen
Open the Entry/Exit Checklist tab.
Below the Add Entry Exit Employee heading
Employee: By selecting employee from the confined space employee select box users can choose the
employee whom they want to add to the confined space entry/exit log.
Click the 'Add Employee to Entry Checklist' button.

Click the 'Plus' of the selected employee to increase the entry log count and change the employee entry status.
Click the 'Minus' of the selected employee to decrease the entry log count and change the employee entry status.

Open the Entry/Exit Checklist tab.

Click the 'Delete' link of the selected employee to delete the entry log for the selected employee.

Open the Personal Protective Equipment Required tab.

30.Submit Confined Space Entry Report |

Once the Confined Space Entry Report details have been entered, the employees added, the list of considerations checked off, the personal protective equipment required added
and the control measures added and the confined space notes reviewed edited to
reflect the work to be preformed, the Confined Space Entry Report should be submitted.
Once on the edit confined space entry report screen
Click the Submit Report button.

31.Close Out Confined Space Entry Report |
|
Once the Confined Space Entry Report details have been entered, the employees added, the list of considerations checked off, the personal protective equipment required added
and the control measures added,the confined space notes reviewed, the employee entry logs completed, and the gas monitoring reading all recorded, the Confined Space Entry
Report should be closed out.
Once on the edit confined space entry report screen
Click the Close Out Report button.

32.Review Confined Space Entry Report |
|
Once a confined space entry report has been closed out, management or supervisory staff should review and sign off that the hazard assessment
has been completed properly and that issues found can be brought to the attention of the proper personnel. Administration can help clients review hazard assessments.
