Disciplinary Reports

The Disciplinary Report section allows administration to help clients to complete disciplinary reports, disciplinary reports need to be created after incidents or issues arise with the work of field staff. Disciplinary reports include a description of the incident or issues with the work of field staff, what actions should and will be taken to improve the situation. These reports can be filed by field staff directly from the IPhone application.

When Disciplinary Reports are submitted to RTPro a email is sent to the companies representative, a copy of the disciplinary report.

R & T Electrics Smartphone application allows field staff to complete first aid kit inspections in the field and submit them to the system. For information about completing a first aid kit inspection through the Smartphone click here.

Configuring Disciplinary Report Submission Email Alert

The company representative that receives a email from RTPro, when a disciplinary report is submitted is a function of who is set up to receive the Disciplinary Report Submission. The person whom should receive this email, should probably be someone in management or human resources. To configure Safety Net for whom receives receives this email alert in the Company Email Alert section.

1.Search Disciplinary Report

Disciplinary Reports can be searched by a combination of employee, supervisor and submission status. Users can narrow search results by choosing any combination of the three search criteria.

All search results can be sorted by clicking on the results headings Date, employee name, supervisor name,offense number, violation type, and action type. Clicking on the same heading can do switching the results order from ascending to descending.

2.Delete Disciplinary Report

Deleting disciplinary reports may have to be done if the disciplinary report was added to the system incorrectly. Only disciplinary reports that have not been reviewed by management can be deleted.

To delete a disciplinary reports click the 'Delete' link of the disciplinary report you want to delete.

3.Add Disciplinary Report

To add a Disciplinary Report, click on the 'Add Disciplinary Report' link.

On the next screen, under Add Disciplinary Report heading, select:

Employee: By selecting a employee from the employee select box users can choose employee who is having the disciplinary report filed against them. On the add disciplinary report you can choose any active employee from the company.

Supervisor: By selecting a employee from the supervisor select box users can choose supervisor whom conducted the disciplinary report. On the add disciplinary report you can choose any active employee from the company.

Then click the 'Add Disciplinary Report' button. The edit first aid kit inspection screen will load. From here, proceed to edit as per the instructions below.

4.Look up Disciplinary Report

To get to the edit screen of a disciplinary report, click on the date of the disciplinary report that you would like to edit.

To view the html version first aid kit inspection report in click on the 'Html view' link of the disciplinary report you would like to view.

From each html view you can also open the PDF version of the report by clicking on the PDF link on the top of the page.

To view a printable document file (PDF) view of the disciplinary report, click on the 'PDF view' link of the appropriate disciplinary report.

5.Edit Disciplinary Report

Once on the Disciplinary Report you would like to edit.

Fields

Employee: By selecting a employee from the employee select box users can choose employee who is having the disciplinary report filed against them. On the add disciplinary report you can choose any active employee from the company.

Supervisor: By selecting a employee from the supervisor select box users can choose supervisor whom conducted the disciplinary report. On the add disciplinary report you can choose any active employee from the company.

Violation Date: The violation date is the day the subject of the disciplinary report occurred. The format of this field is YYYY-MM-DD.

Notice Date: The notice date is the day the disciplinary report and disciplinary action was carried out. The format of this field is YYYY-MM-DD.

Location of Violation: Allows the user to input a description of where violation occurred. The character limit of this field is 500.

Type of Violation: By selecting a type of violation from the type of violation select box users can choose the type of incident that occurred.

Description of Violation: Allows the user to input a description of violation which occurred. The character limit of this field is 500.

Expected Improvement: Allows the user to input a description of expected improvement of the employee. The character limit of this field is 500.

Discipline Type: By selecting a type of discipline from the type of discipline select box users can choose the type of discipline action taken.

Suspension First Date: The suspension first date is the day the employees suspension starts. The format of this field is YYYY-MM-DD.

Suspension Last Date: The suspension last date is the day the employees suspension ends. The format of this field is YYYY-MM-DD.

Click the 'Edit Disciplinary Report Details' button to ensure that any information changed is saved in the system.

6.Add Statement Paragraph

Disciplinary Reports allow users to include Statements, Statements are made up of as many paragraphs required, each paragraph can be moved up or down a statement by changing the order of paragraph value.

To add a new paragraph click, the 'Add Statement'.

Order: By selecting a order value from the order select box users can choose order value of the paragraph statement. Users can move paragraphs up or down a statement by changing the order value of the paragraph.

Click the 'Add Statement Paragraph' button to saved in the system.

7.Look Up Statement Paragraph

Once on the Disciplinary Report you want to edit.

To get to the edit screen of a statement paragraph, click on the edit link of the statement paragraph you would like to edit.

Click the 'Save Statement Paragraph' button to saved in the system.

9.Delete Statement Paragraph

Deleting company first aid kit inspection kit may have to be done if the company first aid kit inspection kit was added to the system incorrectly.

To delete a company first aid kit inspection kit, click the 'Delete' link of the company first aid kit inspection kit you want to delete.

17.Delete Statement Paragraph

Once on the Disciplinary Report you want to edit.

To delete a statement paragraph, click the 'Delete' link of the statement paragraph you want to delete.

18.Submitting Disciplinary Report

Once a disciplinary report has been completed, it must be submitted for review by management. To do this, click on the 'Submit First Aid Kit Inspection' button.

19.Reviewing Disciplinary Reports

Once a disciplinary report has been submitted, management or supervisory staff should review and sign off that the disciplinary report has been completed properly and that issues found can be brought to the attention of the proper personnel.
