Ground Disturbance Plans

When ground needs to be disturbed to complete the job, a Ground Disturbance Plan should be developed. Companies configure the checklist of things that must be checked while completing a ground disturbance plan. Employees will complete the checklist and other details including work description, tools and equipment required, exact location on site and the first call ticket number and date.

Ground Disturbance Plans in SafetyNet require

  1. Job Site: A job site is the identifying where the field staff are working on. In SafetyNet this job site is only for work being done on a single job site.
  2. Field Staff: Field staff whom will be working on the job site need to be active in the SafetyNet system. They will need to
    1. Enter time card entries for time spent working on work order for the day of the ground disturbance plan.
    2. Each employee working needs to be added to the ground disturbance plan and sign off that they have reviewed the ground disturbance plan.*
    3. Companies are recommended to built a company task list**, these tasks will be added to ground disturbance plans when created to cut down the time spent staff need to spend writing out the tasks, task hazards and plans for elimination as part of every ground disturbance plan.

    *While it is possible to add a employee thru the Add Outside Employee feature on the ground disturbance plan, no safety statistics can be created for the employee"s added this way.

Ground Disturbance Plans in SafetyNet track

  1. Safety Details: Safety details about where the work was done on site, what work was done and if any injuries occurred are recorded on a ground disturbance plan.
  2. Employees: Which employees participated in ground disturbance plans and which days.

Safety Net Smartphone application allows field staff to complete ground disturbance plans in the field and submit them to the system. For information about completing a ground disturbance plan through the Smartphone click here.

Once a ground disturbance plan is submitted:

  1. An email can be sent to the person in charge of the ground disturbance plans. (For more information about email alerts check the email alerts section of the manual)
  2. Employee Timecard entries will be updated to show that a ground disturbance plan has been completed for the appropriate job site and day of work.
  3. Safety Net Statistics are updated for number of ground disturbance plans completed for companies, customers, work orders and job sites.

Configuring for Ground Disturbance Plans company task list

SafetyNet allows companies create checklist of what should be reviewed during ground disturbance plan, which are added automatically to ground disturbance plans when they are created.

Management, Office Staff and Safety Officers can edit how company ground disturbance check list in the Ground Disturbance checklist section when they edit the ground disturbance checklist. The ground disturbance plan section is only available to users that have the ground disturbance plan section turned on for their users group. To change users access to ground disturbance plans, management and office staff can change access in Application Section to view the information about the Application Section Click Here

1.Search Ground Disturbance Plan

Ground Disturbance Plans can be searched by a combination of customer, job site, employee and submission status. Users can narrow search results by choosing any combination of the four search criteria.

All search results can be sorted by clicking on the results headings Report Start Date, Report End Date, Customer Name,Job Site Name, Developed By and Status. Clicking on the same heading can do switching the results order from ascending to descending.

2.Delete Ground Disturbance Plan

Deleting ground disturbance plan may have to be done if the ground disturbance plan was added to the system incorrectly. Only ground disturbance plans that have not been reviewed by management can be deleted.

To delete a ground disturbance plan click the 'Delete' link of the ground disturbance plan you want to delete.

3.Look Up Ground Disturbance Plan

To get to the edit screen of a ground disturbance plan, click on the report start date of the ground disturbance plan that you would like to edit.

To view the html version ground disturbance plan report in click on the 'Html view' link of the ground disturbance plan you would like to view.

From each html view you can also open the PDF version of the report by clicking on the PDF link on the top of the page.

To view a printable document file (PDF) view of the ground disturbance plan report, click on the 'PDF view' link of the appropriate ground disturbance plan report.

4.Add Ground Disturbance Plan

To add a Ground Disturbance Plan, click on the 'Add Ground Disturbance Plan' link.

On the next screen, under Add Hazard Ground Disturbance Plan heading, select:

Fields

Report Start Date: The report start date field defaults to today$s date. The date field is formatted in YYYY-MM-DD format and will not accept any other format.

Report End Date: The report end date field defaults to today$s date. The date field is formatted in YYYY-MM-DD format and will not accept any other format.

Job Site: By selecting a job site from the select box, which loads with a job site name users can choose which job site they want to create a ground disturbance plan for.

Employee: By selecting a employee from the employee select box, which loads with the employees first name and last name users can choose whom conducted the ground disturbance plan. On the add ground disturbance plan section you can choose any active employee from the company.

Then click the 'Add Ground Disturbance Plan' button. The edit ground disturbance plan screen will load. From here, proceed to edit as per the instructions below.

5.Edit Ground Disturbance Plan Details

Once on the ground disturbance plan edit screen.

Fields

Report Start Date: The ground disturbance plan report start date is the day the ground disturbance plan was started. The format of this field is YYYY-MM-DD.

Report End Date: The ground disturbance plan report end date is the day the ground disturbance plan was ended. The format of this field is YYYY-MM-DD.

Developed By: By selecting a employee from the developed by select box users can choose which employee on the ground disturbance plan who developed it. Only employee's who have been added to the ground disturbance plan can be made developed it.

Work Description: The Work Description field should be as specific as possible on what work will be completed and what issues exist. The character limit of this field is 350.

Exact Location on Site: The Exact Location on Site field should be as specific as possible, as job sites may include tasks in a number of locations and the Ground Disturbance Plan needs to identify the specific location(s) that were reviewed. The character limit of this field is 500.

Tools to be Used: The Tools to be used field allows field staff to enter the tools and equipment required to complete the work. The character limit of this field is 500.

Notes: The Notes field allows field staff to enter any information important to the job. The character limit of this field is 1000.

Work Permit Required: The Work Permit Required field should be set to Yes if the a work permit required.

Click the 'Save Ground Disturbance Plan' button to ensure that any information changed is saved in the system.

6.Remove Ground Disturbance Employee

Removing a employee may be required if the wrong employee was added to a ground disturbance plan. If the employee did not participate in the ground disturbance plan or did not work on the job site on the given day they should not be on the ground disturbance plan. Once a ground disturbance plan has been submitted employees can t be removed from the ground disturbance plan.

Ground Disturbance Employee: Select from the select box the employee to be removed.

click the 'Remove Employee' button.

7.Add Ground Disturbance Employee

Adding employees to the Ground Disturbance Plan is crucial to reporting accurately. In order to have an accurate picture of the job site, adding employees to the Ground Disturbance Plan needed. While the ground disturbance plan was created, the first employee was added to ground disturbance plan. Adding other field staff to the ground disturbance plan allows a group of workers to share the same ground disturbance plan. Once a ground disturbance plan is submitted all employee's added to the ground disturbance plan will have access to the ground disturbance plan.

Ground Disturbance Employee: Select from the select box the employee to be removed.

click the 'Add Employee' button.

8.Add Temporary Employee to Ground Disturbance Plan

Once and a while a person may need to added to a ground disturbance plan, who is not a employee of your company. A subcontractor, or a customer employee are some examples. Its still important that they review the ground disturbance plan. SafetyNet allows field staff to add outside employees to a ground disturbance plan.

IMPORTANT NOTE Its very important that the add temporary employee function is not used to add a new employee to the company to a ground disturbance plan. Temporary employee statistics are not tracked. So its very important to add a new employee to your company the proper way to SafetyNet. Fields

First Name: The First Name field should indicate the legal given name of the employee. The character limit of this field is 25.

Last Name: The Last Name field should indicate the legal surname of the employee. The character limit of this field is 25.

Email: The Email field should indicate the best (i.e., most often used for business purposes) e-mail address for the employee. The character limit of this field is 25.

Phone Number: The Phone Number field should indicate the best (i.e., most often used for business purposes) telephone number (land line or cellular) for the employee. The character limit of this field is 13.

Company: The Company field should indicate the name of the company for which the employee works. The character limit of this field is 75.

click the 'Add Employee' button.

9.Edit Checklist Details

To edit the checklist details, click on the 'Load Edit Checklist Details' button to load Category checklist. This will load all of the ground disturbance category checklist details. For up to 15 ground disturbance category checklist details the task description will be displayed. The following fields can be changed for each task:

Checked Status: Each task can be marks as NA, Yes or No, change the value by selecting from the select box.

Comments: A comment about the deficiency that requires attention. The character limit of this field is 250.

10.Add Ground Disturbance Image

Once on the edit ground disturbance plan screen

Fields

File Name: Choose a file name Click the 'Choose File' button, the navigate your images to find the image you would like to upload.

Description: Enter the description of the file. The description of the task will be your long description of the file. The character limit of this field is 250 characters.

Click the 'Upload Drawing' button.

11.Look Up Ground Disturbance Image

Once on the edit ground disturbance plan screen

To View the drawing, click on the 'View' link of the drawing that you would like to view.

12.Edit Ground Disturbance Image

To Edit the drawing, click on the 'Edit' link of the drawing that you would like to edit.

Description: Enter the description of the file. The description of the task will be your long description of the file. The character limit of this field is 250 characters.

13.Delete Ground Disturbance Image

Once on the edit ground disturbance plan screen

To Delete the drawing, click on the 'Delete' link of the drawing that you would like to delete.

14.Submit Ground Disturbance Plan

Once the Ground Disturbance Plan details have been entered, the employees added, the list of ground disturbance checklist details edited to reflect the work to be preformed, the Ground Disturbance Plan should be submitted.

Once on the edit ground disturbance plan screen

Click the Submit Ground Disturbance Plan button.

15.Close Out Ground Disturbance Plan

Once the Ground Disturbance Plan details have been entered, the employees added, the list of ground disturbance checklist details edited to reflect the work to be preformed and the work for the given day is completed the Ground Disturbance Plan should be closed out.

Click the Close Out Ground Disturbance Plan button.

16.Review Ground Disturbance Plan

Once a ground disturbance plan has been closed out, management or supervisory staff should review and sign off that the ground disturbance plan has been completed properly and that issues found can be brought to the attention of the proper personnel.

Click the Review Ground Disturbance Plan button.
